Before Surgery

• CR Surgery Clinic Registration form:
  • Please download this form if you would like to print the form, fill it out and bring it to your appointment.
  • Please download this form if you would like to fill this form out on your smartphone or computer and email it back to us at info@crsurgery.com.au. If you are completing it on an iPhone, you need to ‘share to’ the form to the “Books” app (or other form filling app such as “Adobe Fill & Sign”), and when finished, use the ‘share to’ to email it to us. Unfortunately, form filling is not automatically supported on Android phones – you will need to download a free app such as “Adobe Fill & Sign”.
  • You can now complete your registration online using our electronic form. This can be done on any smartphone, tablet or computer.

• Colonoscopy preparation – under 75 years old, no heart/kidney problems or inflammatory bowel disease

Before your admission to hospital – Paperwork

• Cabrini Registration Form – This can either be completed online or on paper:
  • If you wish to complete it online:
    Please click on https://patientportal.cabrini.com.au/ and follow the prompts. If you have any problems or questions with this process, please call Cabrini Patient Services on 9508 1466.
  • If you wish to fill out a paper copy:
    Please print out the Registration Form and complete it. After this, please do one of:
    1) Scan and email it to patientservices@cabrini.com.au OR
    2) Drop them off at the hospital directly OR
    3) Send it via reply paid envelope to the hospital you are having your procedure at (no need for stamps):
    -Cabrini Malvern, Att: Patient Services Department, Reply Paid 63245, Malvern VIC 3144
    -Cabrini Brighton, Att: Patient Services Department, Reply Paid 85198, Brighton VIC 3183.
    
• Cabrini Consent and Medication paperwork
• If it is possible, please print these out and bring it with on your day of admission. The CONSENT (first) page should be filled out in all the sections marked with “*” (star/asterisk). You can find your procedure on the procedure instructions. Please list out all your medications on the MEDICATION (last) page.
• If you cannot print it out, this can be given to you on the day of your admission. Please bring a list of your medications with you. This may cause a short delay in your admission process.
